A customer must submit a valid application. A valid application contains at least the following information:
Applicant's name;
Address or location where the applicant can be reached;
Date the application was signed.
Signature under penalty of perjury (MA1301A.2)
An application may be accepted without a signature to preserve the application date, but it must be signed by an authorized person before MA benefits can be approved.
