PAS is referred

PAS Assessor completes PAS interview as soon as possible, using both electronic tools if needed. See Identifying the Appropriate PAS Tool.

During the interview, the PAS assessor must:

• Explain the interaction between AHCCCS/ALTCS and DDD

• Assist with completing the DDD application

• Get signatures from the appropriate person for the DDD release of information and the AHCCCS release of information

Medical records are returned, and PAS is written up.

Send the DDD referral to the appropriate Intake Referral email box (based on geographic region), and be sure to include all the following documentation:

• Signed DD 524 form

• Signed DD 525 form

• Medical records:

  • Developmental Evaluation

  • Psychiatric Evaluation

  • Psychological Evaluation

  • Medical/PCP records

NOTE     DDD has specific requirements depending on the qualifying DDD diagnosis. They are as follows:

• Autism must be determined by a developmental pediatrician or psychologist, not a school psychologist.

• Intellectual Cognitive Disorder may be determined by a school psychologist.

• Down’s Syndrome requires diagnostic prenatal or postnatal genetic testing results and a report documenting how the practitioner came to the diagnosis based on genetic testing. The testing must be completed by a licensed Primary Care Physician, Developmental Pediatrician, Neonatologist, or Clinical Geneticist.

• Individual School Education Plan(s)- IEP

• Multidisciplinary Evaluation Team (MET) report

• Physical/Occupational/Speech Therapy Evaluations

• Neurological Documentation

• DE-125 Referral to DDD form with the citizenship question answered

DDD makes their decision based on information provided by ALTCS (medical records, application) and returns referral to ALTCS.

Update the DD status in HEAplus and enter the DD or EPD PAS as appropriate.

The Benefits and Eligibility Specialist receives notification of PAS completion and processes application.
