A Notifying SSA When an SSI Cash Customer Enters or Leaves an LTC Medical Facility

Procedures

Follow the steps below when the customer receives SSI-Cash AND:

Is the ALTCS customer currently living in an LTC medical facility or did they leave the facility to go to an HCBS living arrangement?

  • When the customer is in an LTC medical facility, continue to step 2.

  • When the customer moved to an HCBS living arrangement, skip to step 5.

NOTE     When the customer moved from one LTC medical facility to another, update the living arrangement in HEAplus. No further action is needed.

Complete the SSA Notification Form (DE-139). Send the form to the customer’s local Social Security Administration (SSA) office. You can find the SSA office address here.

Enter case notes that the SSA Notification Form (DE-139) was sent to SSA.

Place a copy of the DE-139 in DocuWare.

Make sure the living arrangement is correctly entered and verified in HEAplus.

NOTE     Do NOT change the amount of SSI Cash received in HEAplus to the reduced SSI Cash amount. The HEAplus calculations should automatically do this.

Review the budget worksheets to make sure that the reduced amount of SSI Cash is used (MA606III).

Was the ALTCS customer discharged from an LTC medical facility to an HCBS living arrangement?

  • If YES, update the living arrangement in HEAplus. Continue to step 8.

  • If NO, STOP.

Complete the SSA Notification Form (DE-139). Send the form to the customer’s local SSA office. You can find the SSA office address here.

Enter case notes that the SSA Notification Form (DE-139) was sent to SSA.

Place a copy of the DE-139 in DocuWare.
