Procedure

Review the Notification History page in Health-e-Arizona Plus to make sure a Voter Registration Packet (AH-1001) was sent to the customer.

Use the table below to offer customers the opportunity to register to vote.

Is the customer in person at the local AHCCCS or DES office?

  • If NO, continue to step 2.

  • If YES, ask the customer if they would like to register to vote. If the customer answered the question:

    • No, STOP.  No further action is needed.

    • Yes, take the following actions:

      • Hand the customer the Voter Registration Packet (AH-1001) directly.

      • Tell the customer about the proof of citizenship needed with a voter registration form. Acceptable proof of citizenship for voter registration can be found on page 4 of the voter registration form.

      • When the person has a pending application, use the Voter Registration Format Preference screen in HEAplus to indicate that the Voter Registration form was provided in person. If there is no pending application, no further action is needed.

      • Skip to step 6.

NOTE     The opportunity to register to vote must be offered to any person who visits an office in person even if they are not applying for benefits.

Has the Medical Assistance application been dispositioned ?

  • If YES, continue to step 3.

  • If NO, skip to step 5.

NOTE     When the case has been dispositioned and the customer did NOT get the voter registration packet, mail the customer a packet. Send an email to dmpsescalations@azahcccs.gov with the application ID and a description of the voter registration issue.

Review the Opportunity to Register to Vote question on the initial application, renewal or change of address. If the customer answered the question:

  • No, STOP. No further action is needed.

  • Yes or did not answer the question, continue to Step 4.

Was “Online Using Service Arizona” selected as the preferred method of registering to vote?

  • If YES, STOP. No further action is needed. The voter registration packet will not display in the Notification History because the customer selected to get the information by using Service Arizona.
  • If NO, continue to step 5.

Ask the customer if they would like to register to vote. If the customer answered the question:

  • Yes, ask them how they would prefer to get the voter registration packet and make the selection on the Voter Registration Format Preference screen in HEAplus. The customer will get the voter registration packet via their selected method when the application is dispositioned. Continue to step 6.

  • No, STOP. Make the selection in HEAplus and document your actions in case notes. No further action is needed.

You must offer the customer help in completing the voter registration form. You may NOT:

  • Seek to influence a customer’s political preference or party registration;

  • Display any political preference or party allegiance;

  • Take any action or make any statement to discourage the customer from registering to vote; or

  • Take any action or make any statement that may lead the customer to believe that a decision to register to vote or not to register to vote may affect their services or benefits.

When the voter registration form is returned to AHCCCS, take the following actions:

  • Review the voter registration form and check that:

    • All required questions have been answered; AND

    • The form was signed;

NOTE     When the voter registration form is missing a required answer or a signature, ask the customer to complete the missing information.

  • Ask the customer if they would like AHCCCS to make photocopies of the required proof of citizenship to attach to the voter registration form; and

  • Mail the form and proof of citizenship to the following address:

Document your actions in case notes. If you had to mail a completed voter registration form in step 7, the case notes should contain the following:

  • That you received the voter application from the customer; AND

  • The date and time the form was mailed.
