Procedure

Prior to the customer’s anticipated release date, staff of the detention facility may help the customer apply for benefits.

NOTE Most of these applications are processed by the Research & Analysis R&A Unit at DES. If assistance is required in order to submit an application, please call 602 774-5749.

Follow the steps below when processing an inmate's pre-release application:

Step	Action
1	Complete the application in HEAplus up to the Verification screen.
2	Verify the actual release date with the detention facility staff.
3	Are all eligibility factors, including the actual release date, verified? If YES, STOP. Disposition the application. If NO, continue to step 4.
4	Send a Request for Information RFI to the customer for proof of any pending factors.
5	Did you receive the information requested on the RFI by the due date? If YES, STOP. Verify the pending factors and disposition the application. If NO, STOP. Deny the application using the appropriate reason.
