Step

Action

1

Will the customer turn age 65 before the end of the month of the diary date?

  • If YES, STOP. A DDSA redetermination is not needed.

  • If NO, continue to Step 2.

2

Does the customer have a current SOLQI?

  • If YES, continue to step 3.

  • If NO, request a current SOLQI and continue to step 3.

3

Is the customer receiving Social Security Disability?

4

After the due date, check to see if the customer completed and returned the forms. Were the completed forms received?

  • If NO, STOP. Discontinue eligibility for SSI-MAO or FTW and determine if eligible for any other MA category.

  • If YES, continue to Step 6.

5

Prepare a DDSA packet using the instructions in How to Complete a DDSA Referral. Include the contents of the original DDSA referral packet or copies of the imaged DDSA packet.

6

Review the DDSA decision when received to see if the customer remains disabled or blind. Is the customer still determined to have a disability or blindness?

  • If YES, process the application and update the diary date on the Disability Information page in HEAplus. Document the results in notes.

  • If NO, discontinue eligibility for SSI-MAO or FTW and determine if eligible for any other MA category.
