When the customer cannot provide proof of citizenship, use the table below to help the customer get the proof:

Then... |
Was born in another state or a US territory |
Give the customer the address and phone number for the Vital Records office in their state or territory of birth. Vital Records offices in the US territories and states other than Arizona can be found here. |
Was born abroad to US citizen parents, and the birth was reported to a US consulate or embassy before the customer turned 18 |
Refer the customer to the US Department of State to obtain a replacement Consular Report of Birth Abroad form. See this link for more information on contacting the Department of State. |
Is a naturalized citizen, or was born abroad to US parents and filed an application for citizenship with USCIS after the age of 18 |
Refer the customer to USCIS to obtain documentation of citizenship. The forms and filing fees are available on the USCIS website. USCIS may waive the fee for customers who cannot afford to pay it. See this link for details. When the customer or representative is unable to go to USCIS, send a Document Verification Request form G-845 or G-845S to USCIS requesting verification of citizenship. Attach any available documents that support the customer’s claim to citizenship. See this link for the mailing address for these requests. |
Previously held a US Passport |
Refer the customer to the US Department of State to obtain a copy of passport records. See this link for more information on contacting the Department of State. |
