Procedure

Use the table below when a report is received that a customer moved out of state:

If the report was from...

And...

Then...

The customer

OR

An Authorized Representative or legal Guardian

A forwarding address was provided

The customer’s or representative statement is accepted as proof; Update the physical and mailing address and process the application to discontinue for not an Arizona resident. 

A forwarding address was not provided

Process the change and send the letter to the last known address. 

Anyone else

 

A forwarding address was provided

The customer or customer’s representative must be contacted to confirm the reported change. See Verify Move out of State not reported by Customer or Representative.
