Follow the steps below to enter alternative format requests from the DE-101 or DE-103 application forms.

Did the applicant check the “Yes” box next to the question “Does the customer, authorized representative, or legal guardian have a visual impairment that requires an alternative format for printed letters?”

For the person who requested an alternative format, answer “Yes” to the question “Does this person have a visual impairment that requires an alternative format for printed letters?” on the following screen or screens, as appropriate:

Additional information will appear on the page. Select the alternative format the person indicated on the application. If no option was selected, select “Other”. NOTE If “Other” is selected, you will be prompted to call a toll-free number. You do not need to call this number. |

Click “Next”. Continue with the application registration process.

Did the applicant check the “Visual” box next to the statement “I have an impairment that requires the following accommodations for this application” on page 1 of the FA-001?

Did the applicant check the “Other” box and write in details about a visual impairment of any kind?
NOTE If you are not sure whether the details the customer provided refer to a visual impairment, contact your supervisor. |

For the person who requested an alternative format, answer “Yes” to the question “Does this person have a visual impairment that requires an alternative format for printed letters?” on the following screen or screens, as appropriate:
